Job Title: MDS Coordinator (RN)

Location: Mandan, ND

Shift: Monday–Friday, 8:00 AM – 4:30 PM

Schedule: 13-week contract | 40 hours per week

Setting: Post-Acute / Long-Term Care (LTC)

Position Overview

We are seeking an experienced MDS Coordinator (RN) to support the Resident Assessment Instrument (RAI) and Minimum Data Set (MDS) process in a long-term care environment. The ideal candidate will be detail-oriented, comfortable with assessments, and confident in coordinating with clinical staff to ensure accurate and timely documentation.

Key Responsibilities

• Complete the nursing portions of the MDS and ensure accuracy of all assessment components.
• Sign and validate assessment forms to confirm completion.
• Review MDS records for accuracy, consistency, and compliance prior to submission.
• Collaborate with interdisciplinary team members to gather resident information.
• Observe, assess, and report resident conditions as needed.
• Communicate with providers through EMR systems.
• Maintain resident privacy, dignity, and confidentiality at all times.
• Provide education, support, and guidance to staff regarding the RAI/MDS process.
• Contribute to a safe, organized, and positive resident care environment.
• Follow all nursing and compliance standards, policies, and procedures.

Required Qualifications

• Active RN license in the state.
• Graduate of an accredited school of nursing.
• Minimum 2 years of experience in LTC MDS coordination.
• Knowledge of the RAI/MDS process.
• Ability to communicate effectively in English (verbal & written).
• Basic computer/EMR skills (PCC experience preferred).
• RAC-CT certification required within 6 months (if not already certified).
• Current BLS certification.

• Role involves routine assessments, communication with staff and residents, and use of electronic medical records.
• Must be able to occasionally lift up to 25 lbs.
• Moderate noise level work environment.
