Job Description

A health services network in New York City is actively seeking a Licensed Master Social Worker (LMSW) to join their growing Emergency Room team. In this role, the Social Worker (LMSW) will be responsible for ensuring all high-risk patients are provided with the appropriate social services, as needed.

About the Opportunity:

• Schedule: Full time
• Hours: 1pm to 9pm
• Setting: Emergency Department

Responsibilities:

• Interviews the patients/family to obtain relevant psychosocial information for all patients requiring social work intervention
• Provides psychosocial assessments and discharge planning services as indicated
• Provides concrete services, education, and linkage of patients to appropriate hospital
• Provides motivational counseling, interventions and patient referrals as indicated
• Provides clinical information to HMO's for authorizationsWorks with ACS, HMO's Foster Care Agencies, APS Shelter system and other social service agencies on behalf of patient
• Back up to Inpatient Units as needed
• When indicated and approved by Administrator and Supervisor, provide outreach to family via home visits with assigned coworker
• Provides short term counseling and crisis intervention to all victims of domestic violence child abuse, elder abuse and sexual assault
• Provide bereavement counseling to patients/families as appropriate
• Fulfills departmental requirements for medical record documentation
• Completes monthly statistics and other reports
• Maintains records on patient care planning rounds as appropriate

Qualifications:

• 1+ year of Social Work experience in Adult, Child Welfare, and/or Victim Services
• Master's Degree in Social Work
• An active NYS Licensed Master Social Worker (LMSW) licensure
• Social Work experience in a Hospital setting
• Discharge Planning experience
• Experience working with EPIC EMR or AllScript EMR
